Description of Services for Direct Care Members.
As part of the Direct Care Membership, members receive comprehensive services, including a dedicated primary care supervision by their primary care provider. Importantly, our office does not bill insurance for services rendered to Direct Care Members. However, we recommend securing insurance to cover external procedures, including specialized lab tests, diagnostic imaging (e.g., X-rays and MRI scans), and specialist referrals. The Direct Care Membership package includes the following services:
Direct Access and Enhanced Convenience to Your Healthcare Provider
- Receive round-the clock access to your healthcare provider, 24 hours a day, 7 days a week.
- Utilize unlimited office and virtual consultations, always with your healthcare provider.
- Schedule appointments on short notice including same-day or next-day visits, or drop in for urgent visits.
- Get after-hours medication prescriptions and receive attentive, personalized services from our staff and dedicated healthcare provider, just a phone call or email away.

Prescription Medications and Refills
- Prescriptions will be electronically transmitted to your chosen pharmacy for prompt pickup.

Medical Weight Loss Management Pricing
Initial Free Consultation: This is a free 20 min discovery consultation to understand client's situation and how our services can help them reach their goals.
New Patient Consultation: $285 FOR ALL NEW PATIENTS: This would include 75 min personalized 1:1 video consultation with our medical practitioner, comprehensive health assessment, detailed medical history review, general lab work to confirm suitability for medication.
While standard laboratory tests are covered, specialized tests may require additional fees based on individual health requirement.
A 30 min Free follow up consultation is scheduled for lab result review and personalized treatment plan (Exercise, Diet, Behavioral Therapy Plan & Medications as indicated).
Ongoing Support Include:
- 24/7 access to medical provider
- Expert clinical monitoring personalized progress tracking
- Access to different diet and exercise plans with one specifically for individuals on weight-loss medication.

Follow Up consultations would include: 30 min for weight evaluation to ensure progress is being made. Continuous Exercise, Diet, Behavioral Therapy and follow up labs as indicated
Hormone Replacement Therapy (HRT)/Testosterone Replacement Therapy (TRT)
$350 Initial Consultation fee includes:
Our initial visit is tailored for individuals aiming to enhance their hormones/testosterone levels and sustain optimal health and wellness. This visit would include a comprehensive interview to help us identify the causes of your symptoms, laboratory testing to check hormone levels, individualized treatment plans and medications if appropriate.

$250 per month Follow up Visits includes:
Follow up visits would be individualized. Appointments would be scheduled monthly for the first few months after starting treatments, then every three months, six months and yearly based on individual needs. We would track your progress during each follow up visit to ensure that your symptoms are improving. Follow up labs would be reviewed and medication changes made if needed during this visit. During this visit we would provide the following:
- Monthly injections/medications.
- Supplies needed.
- Follow up labs.
- All supplies and medications would be delivered to your doorsteps.
Quarterly Membership for Hormone Replacement (HRT)/Testosterone Replacement Therapy
$699 billed quarterly after initial consultation and treatment plan if paid in full.
